United Health Care Claims Reconsideration Process

1. Login on UHC portal
2. Search Claim Status (for the concern patient)
3. Go to the bottom of the page and in ACTION Tab click “VIEW OR ACT ON YOUR CLAIM” button.
4.. Go to ASSESSMENT Tab and click “CREATE CLAIM RECONSIDERATION” Button.
5.. It will take you to a new page. Enter/Select all relevant data (also gives you the option to attach a file e.g. HCFA).
6.. Click SUBMIT.
It will give you a Ticket number for future reference.
7.. On MY APPS section select to UHC Claims Reconsideration.
8. Select/Enter provider details.

 

Enter the Ticket number and it will show you the status of you request.
